| STEPS TO SUBMITTING A U.S. PASSPORT APPLICATION FOR ADDITIONAL PAGES: |

| STEP 1: Complete and Submit Form DS-4085: Application For A U.S. Passport |

| (Complete the Online Passport Form DS-4085, then click CREATE FORM button) If for any reason you cannot complete the online application, print the DS-4085 Passport Application and fill out by hand. Also, please do not sign this application until you are instructed to during Authorization process (step #9). After completeting application, continue with the steps below. |
