New United States Passport Requirements |

| For those individuals who: | |
| - have never had a passport | |
| - have lost a valid passport or - whose passport was issued more than 15 years ago or - whose most recent passport is damaged or mutilated or - whose most recent US passport was issued prior to 16th birthday. |

For those who have lost or had their valid U.S. passports stolen, complete Passport Application Form DSP-64. WHAT IS REQUIRED: |

Bring all original materials to a
Passport Acceptance Facility (Post Office) and notify the agent you are using an
expediting service to "hand carry" your application. You will then sign the
passport application form in the presence of the representative who will witness
your signature and place all items (with the exception of the order form)
into an envelope. The representative will then seal the envelope which should be marked "To be Opened by U.S. Passport Office Only". Do not open this sealed envelope or the application will be invalid. Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications". |

as follows: $30 for the post office, $135 ($120 for minors) by check or money order made out to “U.S Department of State, and expediting fee of $150.00 for 24-72 hour service or 100.00 for 10 -14 day service to be paid by credit card only (see order form). Do not write any check until the postmaster or the court clerk tells you to do so. |
