as follows: Check or money order made out to “U.S Department of State for $60.00 if within one year of passport issuance (or $135.00 if the passport was issued more  than
a year and

than 15 years ago - see passport renewal) and expediting fee of $150.00 for
24-72 hour service or 100.00 for 10 -14 day service to be paid
by credit card only (see order form).
