United States Passport Instruction: Passports for Children Under 16

  • Both parents must appear at the Passport Acceptance Facility to sign the application for the minor OR

  •  If one parent is unable to appear, they can provide a signed and notarized Form DS-3053 Statement of Consent OR

  •  One parents appears, signs, and submits primary evidence of sole authority to apply (one of the following):

  1. Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent, or

  2. Court order granting sole custody to the applying parent (unless child's travel is restricted by that order), or

  3. Adoption decree (if applying parents is sole adopting parent), or

  4. Court order specifically permitting applying parent's, or

  5. Judicial declaration of incompetence of non-applying parent, or

  6. Death certificate of non-applying parent.

 
STEPS TO SUBMITTING U.S. PASSPORT APPLICATION FOR A CHILD UNDER 16:
 
STEP 1: Complete and Submit Form DS-11: Application For A U.S. Passport
(Complete the Online Passport Form DS-11, then click CREATE FORM button)

If for any reason you cannot complete the online application, print the DS-11 Passport Application .
and fill out by hand.
Also, please do not sign this application until you are instructed to during Authorization process (step #9).
STEP 2: Submit Evidence of U.S. Citizenship

You must submit the following documents:

  • A certified United States birth certificate OR Original naturalization papers (copies not accepted)

  • Current or expired United States passport if applicable
STEP 3: Present Parents I.D. and Submit a Photocopy of the Identification Document Presented

  • Driver's license

  • Military I. D.

  • Government I. D.


STEP 4: Provide Two Passport Photos

Your Photographs Must Be:

  • Identical
  • In color
  • 2 x 2 inches in size
  • Taken within the past 6 months, showing current appearance
  • Full face, front view with a plain white or off-white background
  • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
  • Taken in normal street attire:
    • Uniforms should not be worn in photographs except religious attire that is worn daily
    • Do not wear a hat or headgear that obscures the hair or hairline
    • If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
    • Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required)
STEP 5: Complete and Submit Two Letters of Passport Authorization
Print and complete two copies of the Passport Authorization Letter

STEP 6: Complete and Submit an Passport Order Form
Print and complete the Passport Order Form
STEP 7: Submit Proof of Travel

  • Copy of Itinerary or Ticket OR

  • Comapny Letter Stating parents job related nee for travel

    Note: If you cannot provide proof of travel, ypur passport will be processed in 5-10 business days.

STEP 8: Passport Fees

Government Fees:

  • Make one check or money order to "US Department of State" for $140.00 (this is the government passport processing fee).

Houston Passport & Visa Service Fees are payable by credit card, check, or money order:

  • Same day - 199.00 + return shipping

  • 24-48 hours - $149.00 + return shipping.

  • 5-10 day - 99.00 + return shipping

STEP 9: Authorization

Both parents should bring the child and all the above listed items to a Passport Acceptance Facility (Post Office or county clerk) and notify the available passport agent that you will be using an expediting service to "hand carry" your application. You will then swear an oath and sign the passport application form in the presence of the representative who will witness your signature and place the following items  in an envelope:

  1. Birth Certificate
  2. Copy of Drivers Licence
  3. Passport Application Form DS-11
  4. Two Passport Photos
  5. Check or Money Order for 170.00
  6. 2 Copies of Passport Authorization letter
  7. Business Letter or Copies of Airline Tickets/Itinerary

*Note: Please DO NOT seal the Order form in the postal envelope!

The representative will then seal the envelope which should be marked "To be Opened by U.S. Passport Office Only".
Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest
that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".
Do not open this sealed envelope or the application will be invalid.
STEP 10: Shipping Your Passport Documents
